    We accept insurance and have a self pay rate. Insurances we accept include: Aetna, Blue Cross Blue Shield, Humana, Medicare, Tricare, Triwest. If you don’t see yours, call our office today. We will look into it for you.

    Our forms are short and sweet. Be sure to bring your ID, insurance cards, and a referral, if you have one, to your first visit. To keep your information safe, we do not have a web based intake form. You may print the forms from the link below or wait to fill them out in the office.

What can I expect at my first visit?

Your first visit to Lystra Physical Therapy will be the evaluation visit with Robert Malone, our physical therapist. He will ask about how your issue began, what makes it worse or better, about your medical history, about medications, and about your goals for physical therapy.

All you need to bring to your first appointment is yourself, identification and insurance cards, and if you have one, your doctors referral.

Do I need a referral for physical therapy?

Yes, after your first visit, we will need a referral from your physician, chiropractor, dentist, surgeon, physicians assistant, or nurse practitioner. If you have a good relationship with your provider and have complained of this issue before, often you can call to request a referral. Other times the provider may wish to see you to rule out more serious conditions causing your pains. We can send our initial evaluation to the physician to help support the need for physical therapy.

TriCare and TriWest patients must have authorizations prior to their first visit. Few other insurances require authorizations, but we will check this when you call.

How long will I have to be in physical therapy and how often?

This is dependent on what you are seeing us for, how committed you are to getting better, your insurance policy limits, and your financial ability to pay for care. Most cases are seen twice a week for 10-12 weeks, but again this is entirely dependent on what you are seeing us for. More complex cases may be seen longer and some only need a few visits.

Our patient population is largely chronic pain patients who may stay with us for a long time. Ultimately, how long you stay in physical therapy is up to you and how consistent you are with your home exercise programs and the adjustments we make to prevent the return of the issues.

What if I can’t afford to pay for my physical therapy?

We know that healthcare can be expensive, especially with the new trend of higher deductible policies. We encourage you to still seek out care because helping you out of pain is more important to us than you paying at each visit.

We offer payment plans of 6, 9, or 12 months so that you can spread your cost over the year and have more peace about your care.

What if I don’t have insurance?

No problem. We have a self pay rate of $90 per visit. You will still need a referral for care after the initial evaluation and you will still get the same high quality care we provide to all patients.
